Apple store account / itunes account
You should have an account to start using the application in apple store. Registration can be done from the phone or by using itunes software or through apple website. Credit card information has to be provided to validate the account. If you select the country as US, then you can validate without providing the credit card details...

Unlock Iphone:
1) If you have purchased iphone from apple store, then you should connect it to the latest version of itunes to unlock. Without unlocking, you will not be able to start using the phone, even with the sim card. Calls / SMS also not possible. Once you unlock using itunes, you can use any provider.
2) If you have purchased iphone, from a provider, then you should use third party software / jail break programs to unlock the phone, so that you can use it with other providers.
